With bitter cold and threat of power outages, Oklahoma’s hospice patients uniquely at risk

Bitter cold and the threat of power outages are especially troublesome to medically fragile hospice patients in Oklahoma and the people who care for them.

Hospice providers in Oklahoma have drawn on their emergency plans for several days, making sure patients were stocked with portable oxygen and other supplies in case they lost power, they said.

“At end-of-life, patients are the most fragile that they'll ever be,” said Jordan Landress, clinical director for Crossroads Hospice and Palliative Care in Oklahoma.

So Crossroads staff has been calling their patients regularly to find out whether they have power, whether they’re warm and whether they have running water, Landress said.

If a patient needs help urgently, “we’re absolutely going to see those patients that need us,” she said.

While some hospice patients may live in facilities like nursing homes, others live at home and may rely on devices like ventilators to survive, said James Garner, president of Kindful Hospice.

“Facilities do have backup generators, they have power, they have a multitude of staff that can help triage during an emergency,” he said. “A patient who lives at home with possibly a single caregiver — whether that's a mother, daughter, son, aunt, uncle, child — the stresses are extraordinary.”

Most families don’t have a backup generator, or a written plan for what to do when temperatures drop into historic lows and the power goes out. That’s why it’s important for hospice care companies to communicate with their patients, he said.

Kindful Hospice encourages families whose loved ones use powered medical equipment — like a ventilator or oxygen, he said — to register with their utility company in order to be prioritized for having service restored in the event of an outage, Garner said.

The Emergency Medical Services Authority, which provides ambulance services in Oklahoma City and Tulsa, said Tuesday that 911 call centers had received an influx of calls from people who rely on powered medical equipment. The call volume was “not sustainable,” EMSA said.

The agency asked people to first call medical providers about a plan for power loss for those with electric-powered medical devices.

“Find out how long meds can be stored at higher temperatures/get guidance for any meds & machines that are critical for life,” EMSA said on Twitter. “As always, any patient experiencing a medical emergency should call 911 immediately.”

Sometimes hospice providers will be the ones on the phone with utility companies, telling them about a patient who cannot lose power in the event of rolling outages, said Tiffany Pine, director of clinical services at Oklahoma Palliative and Hospice Care.

Even with detailed emergency preparedness plans to make sure patients have the supplies and medications they need, providers have to get creative, Pine said. That could mean delivering food for patients and their pets. In over a decade of home care experience, Pine said she's even cut and delivered firewood for patients.

One patient with Oklahoma Palliative and Hospice Care lives in a rural area in Tecumseh, off an unpaved road that’ll be impossible to access until it’s plowed, Pine said. But there are always backup options if the patient needed help urgently, she said.

“We have a tractor, we have a little farm out by Shawnee Lake,” Pine said. “Worst case scenario, we're going to take a tractor ride to get down there.”

Hospice care is more than “helping people die with dignity,” Pine said: it’s all-encompassing.

“We know these patients are dying, but they deserve to die a dignified, comfortable, peaceful death,” Pine said, “not something that is freezing to death in their home.”
